New site - so far have just one category with 4 sub-cats. All re-write correctly to the cat name, except 1 which show catid as 3

When you have non-alphanumeric characters like & which aren't legal in urls it falls back on numeric rewriting. If you changed it to Necklaces and Chains it would use the name rewriting.
