Do I need to upgrade from 3.21 to 3.22 before upgrading to 3.23?

Thanks,

Just follow the upgrade instructions in readme.txt. It tells you how to upgrade from any version, be it 1.00 or 3.21, to the current version.

Quick addon question. I did a manual upgrade of the files from 3.20 to 3.23 because I had to modify a lot of the php files.

Does upgrading supposed to create new tables and modify the database? Should I just simply run the upgrade.php (after following the instructions on what to upload) in that case?

Thanks
David

Upgrade.php is what handles database changes, always.
