I just wanted to know if upgrading to 3.3.21 (from 3.3.19) involves mysql changes or if it's just file updates.

Check the release notes. If it is something major then it should show there, otherwise its usually a file change.

If the upgrade instructions say to run upgrade.php then there was a database change, it they don't then there wasn't.

Release notes only list major new features. An added field isn't important. There was a field added to the attachment table recently to fix a bug of unsubmitted guest attachments showing up to other guests, for example.

Note a typo created a warning on the validation page in 3.3.21, that'll be fixed in about 10 minutes. Anyone who's already upgraded can overwrite classes/onelink.php.
