I have a reciprocal link submitted to my site which is valid and does exist - and works - but the reciprocal link check does not see it.

Does it not like the DIV tag? Or is it the space in the anchor text?

Either it finds your site url value in the page or it doesn't. Either your specified url isn't there, or PHP can't get the page to load.

What would cause PHP to be unable to load the page? It comes up fine in my browser.

I had another link submitted last night and am getting the same thing:

New submission: http://dolphinblog.com
Reciprocal link title on page: "Total Sports Links" (correct)
Reciprocal Link URL: http://dolphinblog.com/
Link goes to: http://www.totalsportslinks.com/NFL/Miami_Dolphins (tested; works)

Link check result:
Checking http://dolphinblog.com/ (link id#3898) for totalsportslinks.com...

Failed. No return link to totalsportslinks.com found on that page.

The page loads fine, the link is there, and it works. What could the problem be?

Question - does the listed reciprocal link have to point to a specific page? Is it required to be http://www.site.com/index.html instead of just http://www.site.com , even if the link is on the default index page?

As I said, it checks if the "My Site URL" value is contained in the page. That is all (well, actually I believe it chops off the http://www. part to be safe in case it's linked without www). Change your site url accordingly if needed.
